Reloadable Debit Card Accounts

• Custodian must compose a Letter of Justification explaining the purpose of the study and how it is linked to the project.

• Prepaid Administrative Website User Request Form
• Memorandum of Understanding (MOU) for reloadable debit card accounts:
  • Custodian
  • Reconciler
  • Coordinator

You will need to use the NR Number Generator tool to generate a unique number for your new reloadable debit card account.
